Student Forms
Students selected for verification will have required documents assigned in their To-Do List in MySJSU. Student Forms is used to submit documents requested to complete the verification process. Submit the required documents as soon as possible to avoid delays in processing financial aid.
How to Create a Student Forms account:
- Log in to sjsu.studentforms.com
- Enter student information exactly as it appears on FAFSA or CADAA. For CADAA applicants,
use your CSAC ID number instead of a social security number.
IMPORTANT:
- The information submitted must be exactly as it appears on your financial aid application.
- CADAA applicants will use their CSAC ID number as a social security number.
